Information on filling out the library card

The application (PDF file) can be filled out online, the fields that can be filled out as well as the mandatory fields are marked. The following browsers are recommended for this:

» Microsoft Edge (current version)
» Google Chrome (current version)

Depending on the browser settings, Mozilla Firefox may have problems with online completion. In such cases, please download the application and open and complete it using e.g.  Acrobat Reader DC.

If you fill out the application by hand, please do so legibly and in block letters. Please also make sure that the first and last names and the e-mail address are legible (the access data will automatically be sent to this address).

For submission by e-mail, please scan the application and an official photo ID1 and submit it in the form of a PDF file or graphic file (JPG, GIF, etc.).
